NOTICE OF PRIVACY PRACTICES

Effective Date: June 1, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU MAY ACCESS THIS INFORMATION, AND YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION. PLEASE REVIEW IT CAREFULLY.

This Notice applies to all protected health information ("PHI") maintained by Little Dragon Integrative Medicine LLC ("Practice," "we," "our," or "us").

YOUR RIGHTS

You have the following rights regarding your health information:

Right to Inspect and Obtain Copies

You have the right to inspect and obtain a paper or electronic copy of your medical record and other health information that we maintain about you.

To request access to your records, submit a written request to our office.

We will provide access within thirty (30) days of your request. If additional time is needed, we may extend this period by up to thirty (30) additional days and will notify you in writing.

We may charge a reasonable, cost-based fee permitted by law for copies, supplies, postage, or preparation of summaries.

If records are requested for purposes required by law to be provided without charge, no fee will be assessed.

Right to Request Amendment

You may request correction of health information that you believe is inaccurate or incomplete.

Requests must be submitted in writing.

If we deny your request, we will provide a written explanation and inform you of your right to submit a statement of disagreement that will become part of your record.

Right to Confidential Communications

You may request that we communicate with you in a particular way or at a specific location.

Examples include:

  • Calling only a specific phone number

  • Sending mail to an alternate address

  • Limiting voicemail messages

  • Restricting email communications

We will accommodate reasonable requests as required by law.

Right to Request Restrictions

You may request restrictions on certain uses and disclosures of your health information.

While we are not required to agree to every request, we will comply when required by law.

If you pay for a healthcare service or item completely out-of-pocket and request that information related to that service not be disclosed to your health insurer, we will honor that request unless disclosure is otherwise required by law.

Right to an Accounting of Disclosures

You have the right to receive a list of certain disclosures of your health information made by us during the six (6) years prior to your request.

One accounting per twelve-month period will be provided without charge.

Additional requests may be subject to a reasonable fee.

Right to a Copy of This Notice

You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

Right to Designate a Representative

If you have granted a healthcare power of attorney or have a legally authorized representative, that individual may exercise your rights and make decisions concerning your health information to the extent permitted by law.

Right to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with:

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.

You will not be retaliated against for filing a complaint.

OUR USES AND DISCLOSURES

Treatment

We may use and disclose your health information to provide, coordinate, and manage your healthcare.

Examples include:

  • Acupuncture treatment

  • Herbal medicine consultations

  • Communication with other healthcare providers involved in your care

  • Telehealth services provided through secure platforms such as Doximity

Payment

We may use and disclose your health information to obtain payment for services provided.

This may include:

  • Insurance billing

  • Verification of insurance benefits

  • Collection of payment

Healthcare Operations

We may use and disclose health information for healthcare operations, including:

  • Quality assessment and improvement

  • Staff training

  • Business management

  • Legal and accounting services

  • Compliance activities

Business Associates

We may share information with trusted third-party service providers who assist in operating our practice and who are legally required to safeguard your information.

Examples may include:

  • Electronic health record providers

  • Billing services

  • Information technology providers

  • Legal and accounting professionals

Appointment Reminders and Health-Related Communications

We may contact you regarding:

  • Appointment reminders

  • Follow-up care

  • Treatment recommendations

  • Healthcare-related benefits or services

Communications may occur through:

  • Telephone

  • Voicemail

  • Text message

  • Email

While reasonable safeguards are used, electronic communications may involve privacy and security risks beyond our control.

Educational Communications and Newsletters

From time to time, we may send educational materials, wellness information, newsletters, or announcements regarding services offered by the Practice.

You may opt out of these communications at any time.

Individuals Involved in Your Care

Unless you object, we may share relevant information with family members, caregivers, or others involved in your care when appropriate.

Student Observers

Students, observers, or healthcare professionals in training may occasionally observe treatment for educational purposes only with your knowledge and permission.

You may decline the presence of an observer at any time without affecting your care.

USES AND DISCLOSURES REQUIRED OR PERMITTED BY LAW

We may use or disclose your information as required or permitted by law, including:

  • Public health reporting

  • Reporting abuse, neglect, or domestic violence

  • Health oversight activities

  • Law enforcement requests

  • Judicial or administrative proceedings

  • Workers' compensation claims

  • National security activities

  • Organ and tissue donation activities

  • Coroner and medical examiner functions

  • Threats to health or safety

AUTHORIZATIONS

Except as otherwise permitted or required by law, we will not disclose your information without your written authorization.

Written authorization is required for:

  • Marketing activities where required by law

  • Sale of protected health information

  • Most disclosures of psychotherapy notes

  • Other disclosures requiring authorization under HIPAA

You may revoke an authorization in writing at any time.

INCIDENTAL DISCLOSURES

Despite reasonable safeguards, incidental disclosures may occur during the normal course of providing healthcare services.

Examples may include:

  • Conversations that are unintentionally overheard

  • Sign-in sheets

  • Calling patients by name in waiting areas

We maintain safeguards designed to minimize these occurrences.

BREACH NOTIFICATION

We are required by law to maintain the privacy and security of your protected health information.

If a breach occurs involving your unsecured protected health information, we will notify you without unreasonable delay and no later than sixty (60) days after discovery, as required by law.

42 CFR PART 2 INFORMATION

If we receive records subject to federal substance use disorder confidentiality laws (42 CFR Part 2), those records will receive the additional protections required by applicable law.

Such records will not be disclosed except as specifically permitted by law or authorized by the patient.

MAINE CONFIDENTIALITY PROTECTIONS

Certain categories of information may receive additional protection under Maine and federal law.

These may include:

  • Mental health information

  • HIV/AIDS and sexually transmitted disease information

  • Genetic testing information

  • Substance use disorder treatment information

  • Other specially protected health information

Where Maine law provides greater privacy protections than HIPAA, we will comply with the more protective requirements.

CHANGES TO THIS NOTICE

We reserve the right to change this Notice and make revised provisions effective for all protected health information maintained by the Practice.

Current versions will be available at our office and on our website.
